ShareCare: Direct - Enabling 'Self directed care'
ShareCare: Direct supports the 'personalisation' of adult social care by providing a comprehensive set of online 'self-directed care' services.
ShareCare: Direct has been developed to support the ‘personalisation’ of adult social care. It has received national funding, was created in association with local authority partners and provides a comprehensive set of online ‘self-directed care’ services.
ShareCare: Direct delivers a dedicated one stop service, enabling members of the public to:
- locate relevant information and advice;
- undertake online assessments;
- search a local marketplace of service providers;
- identify services that can meet their needs;
- submit enquiries and assessment requests;
- communicate directly with care providers and professionals;
- manage their own care plans;
- and share information with professionals, carers and other family members.
The ShareCare: Direct service is complemented by the ShareCare: Adult Services Directory and can be integrated with local health and social care business systems through the ShareCare Integration Hub.
In addition to the Self Directed Support functionality the following software modules are also supported by the ShareCare: Adult system: Adult System; Single Assessment Process SAP and NHS Personal Demographics Service integration.
