Cheshire County Council Single Assessment Process (SAP) for Older People is a partnership project between Health and the County Council providing a web based secure solution for practitioners to complete various mobile or office based online assessments, to share this information where appropriate and to manage and audit track case referrals whenever further action/intervention is required.
